For those applying for SISTAD courses.
This should be used for all SISTAD courses except the following:
- Award in Education & Training
- BA (Hons) Christian Theology & Practice
- Certificate in Employability Practice
- Certificate in Pastoral Supervision
- Certificate in Coaching and Mentoring
Application Form 2
For those applying for external training e.g. Certificates, Diplomas, Degrees or SNVQs and the following SISTAD courses:
- Award in Education & Training
- Certificate in Employability Practice
- BA (Hons) Christian Theology & Practice
- Certificate in Pastoral Supervision
- Certificate in Coaching and Mentoring
Application Form 3
For those applying for Post Graduate, Masters and Doctorate level courses run by external providers
Apprenticeship Application Form
This should be used for all Apprenticeship applications. Send to [email protected] when completed
Group Application
Form
This form should be used by THQ, Divisions or Regions when applying for training for a number of people at one time. It should not be used for groups undergoing training that is accredited. These courses should be applied for by individuals using either application form 2 or 3 as appropriate.

For those applying for any Open Learning course.
